How did three nurses make drug administration errors?

As a mother, my heart aches for the families of the babies who died at Methodist Hospital. I am also a registered nurse, and my gut aches for the medical staff involved in the events. I must ask how it is that five nurses, not just one, made this error when the cardinal rule in administering a medication is: First, read the label on the vial as you pick it up; second, read the label on the vial as you draw up the medication; and finally, read the label on the vial as you replace it in the drawer.
